Please fill out the this inquiry form and include in your comments about your medical situation the following:

1.  Medical condition: what's wrong and how did you get this way?

2.  Healthcare Provider: include the names and titles of anyone who you think may have done something wrong including the hospital or clinic.  What do you think they did wrong?

3.  Diagnosis/Prognosis/Course of Action: what does your healtcare provider say your condition is and what can you look forward to concerning your injury or condition?

4.  When did this happen to you?  When did you learn what had happened?
